As a Telephone Triage Registered Nurse, you will be a compassionate and reassuring voice for hospice patients and their families, providing guidance and support over the phone. Using active listening and skilled questioning, you’ll assess patient needs, coordinate nurse visits, and arrange for medications, supplies, or medical equipment as needed. In this role, you’ll document encounters, offer hospice instructions in line with established protocols, and collaborate closely with patients, families, caregivers and hospice staff to ensure seamless care, helping to bring comfort and peace of mind in every interaction.

This is a remote PRN position after orientation. Hours and days vary between 8:00am to 6:30pm or 12:00pm to 10:30pm, weekdays and weekends. Training will be at least 3 days a week Monday - Friday, 8a-5p, 6-8 weeks long, on-site, at the Business Operations Center in Independence, Ohio. You have to live within 1 hour of Independence, Ohio.

A caregiver who excels in this role will:

  • Utilize active listening and questioning to provide over-the-phone assessments.
  • Effectively triage and provide resolution to all hospice department telephone calls regarding active or referred patients.
  • Accept verbal telephone orders from physicians to add/change services being provided to active patients and processes accordingly in the system.
  • Assess patient needs, determine and initiate appropriate action or response to meet identified needs.
  • Assess physician needs, provides requested information and/or guidance or service as appropriate or forward to the appropriate person on the management team.
  • Initiate and independently complete appropriate follow-up activities, including appropriate communication with patient/caregiver, agency staff, physician (as applicable) and complete documentation of events and interventions.
  • Communicate as applicable to field staff, office staff and physicians.
  • Assist, review, research and resolve active patient and referral complaints as well as record outcomes accordingly to meet regulatory compliance standards.

Minimum qualifications for the ideal future caregiver include:

  • Graduate of an accredited school of professional nursing
  • Current state licensure as a Registered Nurse (RN)
  • Minimum one year of nursing experience in med/surg, long-term care, home care and/or hospice
  • At least 6 months of hospice experience

Preferred qualifications for the ideal future caregiver include:

  • Bachelor’s of Science in Nursing (BSN)
  • Previous telephone triage experience

Physical Requirements:

  • Manual dexterity to operate office equipment.
  • Requires good visual acuity through normal or corrected vision. Must be able to hear normal conversation.
  • Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
