Coordination and continuity of care are central to this role. As a Nurse Practice Manager for Interventional Cardiology patients, you collaborate closely with primary care providers, referring physicians, cardiology teams, surgeons, fellow nurse practice managers, and other healthcare professionals to address both pre- and post-surgical needs. Responsibilities include reviewing clinical information with surgeons, scheduling appropriate tests and appointments, directing additional studies to outpatient scheduling as needed, and facilitating hospital transfers in partnership with scheduling officers and surgeons’ offices to ensure a seamless, patient centered surgical experience from referral through recovery.
A caregiver in this position worksdaysfrom 8:00 a.m. -- 4:30 p.m.
A caregiver who excels in this role will:
Manage the intake process for surgical candidates by obtaining clinical informationrelativeto thepatientstate of health and level of functioning.
Ensure departmental compliance with regulatory andThe JointCommission requirements.
Coordinate with the PCPs, referring physicians, patients, CCF Cardiology staff, surgeons, office staff, otherNPMs,scheduling officersandpreop/TCI/postopNurseClinicians to assure continuity of care andappropriate concernfor individualized presurgical and/or postsurgical needs.
Review all clinic information available for review with surgeon forappropriate schedulingand communicateadditionalstudies to the outpatient scheduling office under physician direction.
Facilitatehospital transfers in collaboration with scheduling officers, access plus and surgeonoffice.
Minimum qualifications for the ideal future caregiver include:
Bachelor’sof Science in Nursing (BSN) from an accredited school of Professional Nursing
Current state licensure as a RegisteredNurse (RN)
Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
Four years of experience as an RNin Medical Cardiology or Cardiac Surgery clinical areas
Preferred qualifications for the ideal future caregiver include:
Ambulatory background
Care coordinator background
Physical Requirements:
Requires the ability to walk to various locations throughout the foundation, normal or corrected vision and hearing to normal range and the ability to work and function in a stressful environment.
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
Personal Protective Equipment:
Follows standard precautions using personal protective equipment as required.
