The Lead Pharmacy Technician – Medication Historian supports inpatient pharmacy operations at Cleveland Clinic Akron General Hospital, helping ensure medication accuracy, patient safety, and continuity of care within a fast-paced acute care environment. This role is responsible for conducting detailed medication history interviews with patients and families, identifying and resolving medication discrepancies, and collaborating with providers, retail pharmacies, and interdisciplinary caregivers to maintain accurate medication records upon admission. In addition to medication reconciliation responsibilities, this caregiver also supports operational pharmacy workflows including carousel and Pyxis functions while partnering closely with pharmacists and clinical teams on medication safety initiatives. This position offers a collaborative team structure with another caregiver currently serving in the role.

A caregiver in this position will work Monday through Friday from 9am-5:30pm or 10am-6:30pm with every 3rd weekend rotated. No holiday, on-call (at this time) or floating requirements.

A caregiver who excels in this role will:

  • Compile a complete and accurate list of targeted patients home medications including over the counter drugs and herbal supplements.
  • Document drug name, dosage, frequency, compliance, and last dose taken of the patients home medications.
  • Interview patients, family members, and caregivers to obtain a complete and accurate list of drug allergies and their associated reactions.
  • Document the medication history and allergy information in the electronic medical record accurately and in a timely manner.
  • Proactively verify and document patients drug benefit information within the patient chart upon admission to the hospital.
  • Prioritize workload based on patient acuity and volume of admissions.
  • Use available resources to reconcile the patient's home medication list by communicating with physicians, pharmacies, nursing homes, payers, etc.
  • Notify the responsible pharmacist of any omission, duplication, error, non-formulary medication, or other important details related to the patients home medication list.
  • Mentor and assist with training new Medication Historians and pharmacy learners.
  • Must work with the Medication Safety Coordinator and the Emergency Department Pharmacists to perform continuous quality improvement on the medication reconciliation process. Must detect, report, and resolve any concerns in a timely manner.
  • The Medication Historian works in accordance with established practices, procedures, and administrative policies of Akron General Medical Center.
  • Continually demonstrates behavior that anticipates and meets the needs of patients, visitors, and other customers both externally and internally.
  • Produces quality results which provide caring and excellent health services that are progressive, patient-centered, and family-oriented.
  • Demonstrates effective methods for cost reduction.
  • Meet with newly admitted patients and/or families to educate and inform them about the existence and value of the discharge medication concierge program.
  • Communicate effectively with caregivers through various means including, but not limited to: the electronic medical record system, paging system, telephone, and direct personal communication.
  • Assist with the development and implementation of policies and procedures related to the admission medication reconciliation process.
  • Completes mandatory education and training in order to maintain organization and department specific competencies and requirements.
  • Follows health-system and department policies and procedures with special attention to attendance, punctuality, confidentiality, dress code, display of ID badge, and safety.
  • Attend and participate in pharmacy staff meetings.

Minimum qualifications for the ideal future caregiver include:

  • Associate degree in a related field or the equivalent including courses in chemistry, and math
  • Certified Pharmacy Technician (CPhT) verified through the Pharmacy Technician Certification Board (PTCB).
  • Upon hire, registration as a Certified Pharmacy Technician, Registered Pharmacy Technician or Pharmacy Technician Trainee with the Ohio Board of Pharmacy. Registration as a Certified Pharmacy Technician with the Ohio Board of Pharmacy is required within 6 months.
  • Previous experience in a pharmacy or the equivalent; demonstrated interest, aptitude, and ability for pharmacy work; good attention to detail

Preferred qualifications for the ideal future caregiver include:

  • Graduate of an accredited certified pharmacy technician training program


Physical Requirements:

  • Ability to perform work which requires constant standing and walking.
  • A willingness to perform work which has infrequent disagreeable features and safety hazards.


Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required
